Michigan Community VNA Home Health and Hospice, a Detroit-based provider of skilled home health, therapies, palliative and hospice services, has been acquired by Encore Management Group-backed LiveWell Partners.

The transaction represents the platform’s third Michigan acquisition and extends its footprint into the Lansing market, joining LiveWell locations across Illinois, Kansas, Michigan, Missouri and Ohio.

“Joining LiveWell Partners allows us to strengthen the mission we’ve built in Detroit for decades while gaining the resources of a larger platform,” said Vicki Welty, CEO of Michigan Community VNA, in a statement.

The acquired company will retain its brand and leadership under a model that gives local agencies access to shared clinical and operational resources.

CEO Stacie Bratcher noted that VNA complements LiveWell’s John Pail Home Health business.
