Included Health acquires Firefly Health

Carlyle-backed Included Health agreed to acquire Firefly Health, a technology-enabled employer health plan built around advanced primary care.

Firefly serves more than 20,000 members nationally and brings a network of over 2,300 in-person, in-home and specialty care providers.

The company said its administrative-services business generated more than 15% lower total cost of care in 2025 compared with risk-adjusted benchmarks, with every employer customer reporting savings.

The transaction is expected to close in the third quarter of 2026, subject to regulatory approvals.

Included Health plans to combine Firefly's health plan with its virtual care, navigation and care-coordination platform.
